I am the newly and first OT hired for an adolescent inpatient unit in Boston. I will developing OT service on a very established and comprehensive unit. I am looking for sample OT evaluations for this specific population or others interested in giving me feedback on what my OT eval should look like, etc. The children are 8-17y.o. and are very acute (psychosis, harm to themselves or others, AN very low body weights, OCD, ADHD, Autism, anxiety, bi-polar, etc). Any contacts or comments would be welcome.

Shelley,
I also work on an in-patient child/adolescent psychiatric unit. One tool that I have recently started using is the Child Occupational Self-Assessment. I have found it to be one of the most effective tools I have tried for gathering information from this population. You might want to give it a try.
